Welcome to Atlas Community Care. These Terms of Service ("Terms") govern your use of our website and services. By accessing or using our website at atlascommunitycare.com or enrolling in our services, you agree to be bound by these Terms.

Our Services

Atlas Community Care is a Montana Medicaid-approved provider of Personal Assistance Services (PAS) and Community First Choice Services (CFCS). We provide:

  • Family caregiver enrollment and support
  • Self-directed care coordination
  • Training and ongoing education for caregivers
  • Administrative support and compliance management
  • Nurse supervision and care planning

Eligibility and Enrollment

Member Eligibility

To receive services through Atlas Community Care, members must:

  • Be enrolled in Montana Medicaid
  • Be approved for PAS or CFCS services
  • Have a documented need for in-home support
  • Complete the enrollment process with accurate information

Caregiver Requirements

Family caregivers must:

  • Be at least 18 years of age
  • Pass a background check
  • Complete required training
  • Not be the legal guardian or spouse of the member (per Montana Medicaid rules)
  • Comply with all Montana Medicaid PAS/CFCS requirements

Your Responsibilities

When using our services, you agree to:

Provide accurate and complete information during enrollment

Update us promptly of any changes to your contact information, health status, or care needs

Follow all care plans and service agreements

Accurately complete timesheets and EVV documentation

Comply with all Montana Medicaid program rules and regulations

Maintain confidentiality of login credentials and account information

Use our website and services only for lawful purposes

Payment and Billing

Our services are funded through Montana Medicaid programs. There is no cost to eligible members for approved services. Caregiver payment is processed based on:

  • Approved service hours in the member's care plan
  • Accurate timesheet submission and EVV verification
  • Compliance with Montana Medicaid billing requirements
  • Timely submission of all required documentation

Important Note

Fraudulent billing, false documentation, or failure to provide services as documented may result in termination of services and potential legal action.

Termination of Services

Either party may terminate services with appropriate notice. Atlas Community Care reserves the right to terminate services immediately if:

You violate these Terms of Service

You provide false or fraudulent information

You fail to comply with Montana Medicaid requirements

Member eligibility for Medicaid services ends

Safety concerns arise regarding care delivery

Members may also choose to transfer to another agency at any time per Montana Medicaid freedom of choice provisions.

Limitation of Liability

To the fullest extent permitted by law, Atlas Community Care and its staff, officers, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from:

  • Use or inability to use our website or services
  • Errors or omissions in website content
  • Unauthorized access to your account or information
  • Actions or inactions of caregivers outside our supervision
  • Technical failures or service interruptions

Atlas Community Care provides administrative oversight and support but does not assume liability for all aspects of care delivery. Members and caregivers maintain responsibility for safe, appropriate care.

Intellectual Property

All content on this website, including but not limited to text, graphics, logos, images, and software, is the property of Atlas Community Care or its content suppliers and is protected by copyright, trademark, and other intellectual property laws.

You may not reproduce, distribute, modify, or create derivative works from any content on this website without our express written permission.
