Atlas Community Care provides Medicaid-funded Personal Care Services that support individuals with daily living needs in their home or community setting. Our services include:
Clients may choose a family member to be their paid caregiver (when allowed by their Medicaid program or waiver).
Clients may choose their caregiver, set their schedule, and participate in their care planning.
These are NOT services we provide, but state Medicaid programs that fund PCA or family caregiving services.
State Plan Personal Care Services (PCS)
In-home personal care funded directly through Medicaid.
We accept clients who receive services through the following South Dakota Medicaid waivers:
CHOICES Waiver
(Adults with disabilities)
HOPE Waiver
(Elderly individuals requiring long-term support)
Family Support 360 Waiver
(Children with disabilities; family caregiver allowed)
Community Support Waiver
Assistive Daily Living Services (ADLS) Waiver
Home-Based Services Waiver
Important Note:
These waivers determine eligibility. Atlas Community Care provides the personal care services funded through these programs—we do not administer or process the waiver itself.
When a client already has one of the accepted Medicaid
waivers or State Plan PCS:
There is no waitlist with Atlas Community Care.
We can onboard clients as soon as their referral or authorization is confirmed.
Services can begin in as little as 5–14 days depending on the program.
If a client is not yet enrolled, they will first need a state assessment for eligibility.
