Eligibility varies by state.
A nurse will visit you at home (or virtually) to determine if you're eligible for Medicaid-funded in-home care. You'll find out how many weekly hours of care you can get.
You choose a caregiver: your daughter, son, relative, or friend. We come to your home to enroll you in the program.

We do not sell, trade, or rent your personal information to third parties. We may share your information with: state Medicaid agencies as required for program enrollment, healthcare professionals involved in your care, and as required by law or legal process.
By providing your phone number and consenting on our forms, you agree to receive calls and text messages from Atlas Community Care regarding enrollment, care coordination, and service information. Message and data rates may apply. You may opt out at any time by contacting us.

By opting in to receive text messages from Atlas Community Care LLC, you consent to receive recurring automated text messages related to enrollment, care coordination, appointment reminders, and service updates. Messages may be sent using an automatic telephone dialing system.
Message frequency varies. You may receive up to 10 messages per month depending on your enrollment status and care needs.
You can opt out of receiving text messages at any time by replying STOP to any message. After opting out, you will receive a confirmation message and will no longer receive texts from us. Opting out of texts does not affect your enrollment or services.
Message and data rates may apply depending on your wireless carrier and plan. Atlas Community Care is not responsible for any charges incurred from your carrier.

Enrollment in our programs is subject to Medicaid eligibility requirements as determined by each state's Medicaid program. Atlas Community Care does not determine Medicaid eligibility. We assist with the enrollment process for eligible individuals.
You agree to provide accurate, current, and complete information when filling out forms on our website. Providing false or misleading information may result in denial of services.
